AVQA flagged a strange pattern: perfect voice quality during calls, but a MOS of 1.0 during idle times. Drilling down revealed the gateway sent non-standard keep-alive packets, breaking the sequence numbering RFC 3550 and ED-137 require. Radios saw this as massive packet loss. A clear compliance violation, caught before it caused real harm.
